Physicals

Pre-Participation Sports Physicals are required by The Walker School and the Georgia High School Association.  All student-athletes must submit an updated physical form prior to participation in athletic activities including summer workouts/skills sessions, tryouts, official practices and games.  See below for additional information.  


  •  Physicals forms must be dated APRIL 1, 2023 or later to meet submission requirements for the 2023-2024 school year.  

  • Physicals must be submitted on this GHSA form. No substitutes will be accepted and ALL 4 pages must be included.  

  • All physical forms should be uploaded to your Walker Web account.  For instructions click HERE
