Physical Form
All MS and US athletes must have this GHSA physical form on file to be eligible to participate in Walker athletics. Some Publix and Walgreens locations offer physicals for athletes.
MS Athletic Policy and Transportation Form/US Athletic Policy and Transportation Form
All Walker athletes and their parents are encouraged to review and sign the policy forms and turn in to their respective coaches. Walker parents must sign and turn in the transportation form to approve your son or daughter to ride home or to contests with non-family members.
NOTE: Parents and/or student-athletes can turn in forms to Coach Blohm's office in Coles Athletic Center. There is a drop box outside his office you may place them in if the door is closed.
Player/Program Separation Policy
1. Walker athletes that begin a sport then quit the team after 2 weeks of practice or after the team's first contest will not be allowed to participate in a Walker sport during the same season or the season immediately following.
2. Athletes may file an appeal in instances of extraordinary circumstances through the Athletic Director's office.
